In recent times, numerous employees have experienced issues with the disabling of their Universal Account Numbers (UAN). When visiting the EPFO for UAN activation, the department requires the submission of Form-5 and Form-10 as part of the UAN reactivation process. These forms must be issued by the employer.

Furthermore, during the verification of establishments that benefited from the Aatmanirbhar Bharat Rozgar Yojana (ABRY) scheme, the department has requested these forms. However, these forms are irrelevant since the implementation of UAN and online ECR has streamlined and replaced such manual filings. The EPFO department has also issued advisories indicating that Form-5 and Form-10 are no longer required for submission.

EPF FORM-10

Purpose:

  • To report the details of members who have left employment or service during a specific month.
  • Applicable under both the EPF Scheme, 1952 [Paragraph 36 (2) (a) & (b)] and the Employees’ Pension Scheme, 1995 [Para 20 (2)].

Key Points:

  1. Includes information about employees who are no longer part of the establishment due to resignation, termination, retirement, death, etc.
  2. Submitted monthly to the EPFO by the employer.

EPF FORM-5

Purpose:

  • To report details of employees who have become eligible for EPF, EPS, and EDLI membership for the first time during a specific month.
  • Applicable under the EPF Scheme, 1952 [Paragraph 36 (2) (a)] and the Employees’ Pension Scheme, 1995 [Para 20 (4)].

Key Points:

  1. Tracks new employees who meet the eligibility criteria (e.g., earning up to ₹15,000/month as per existing rules).
  2. Submitted monthly to the EPFO by the employer.
  3. Ensures timely registration of employees under EPF, EPS, and EDLI schemes.
